Overview

Southlake Residential Care Village ( Southlake Village) is a new five-storey long-term care facility that opened in December 2004. It is located at 640 Grace Street on the south-east section of the Hospital's property next to Sunnyhill Park in Newmarket.

Southlake Village is a not-for-profit, charitable corporation that employs approximately 200 full-time and part-time staff. Although Southlake Regional Health Centre was closely involved in the formation of the Southlake Village corporation in 2002, it is a separate legal entity that is responsible for its own day-to-day operations and accountable to its own Board of Directors.

Welcome To Southlake Residential Care Village

Southlake Village is a Ministry of Health & Long-Term Care approved, fully accredited 192-bed nursing home. As a nursing home, we provide 24-hour a day nursing and personal care to people who are unable to live on their own, or who require a secure and protective environment.

We recognize that the decision to place a family member in a long-term care facility or nursing home can be very stressful and traumatic on everyone in the family. Many of us have been through this process ourselves. We know, first-hand, how important it is to feel confident about the information we receive and to feel reassured by professional staff who are approachable and caring.

To assist you and your family in making the best possible decision, we are pleased to offer this information which is designed to assist you in understanding the many features of our nursing home. However, if additional questions do arise, please feel free to contact us at any time. Our team is very experienced and many of our staff members have given over 20 years of dedicated service to our residents. If required, an experienced Social Worker is available to provide advice and to answer your questions. Members of our Family Council are also available to provide peer support.

At Southlake Residential Care Village, our mission is to provide the highest quality health care services in a safe and caring environment for those entrusted to our care.

Vision

To provide a home-like atmosphere in which each individual may maintain a sense of self-esteem and identity, find enjoyment and satisfaction in living, and dignity and peace in dying.

Benefits Of Accommodation

Private Accommodation
For people who treasure their personal space, private accommodation offers the greatest amount of privacy and individual space. You will receive a private room and private bathroom. Furnishings include a bed, a wardrobe, bedside table, 3-drawer chest, and comfortable chair.

Standard Accommodation
Standard accommodation rooms have been designed for maximum privacy. In this arrangement, two residents share a washroom and share the vestibule between separate bedrooms. As with the private room, you will receive a bed, a wardrobe, bedside table, 3-drawer chest, and comfortable chair.

We encourage our residents to personalize their rooms and to create a space that truly belongs to them. To assist our residents, a locked “memory box” is provided outside each room in which photos and personal momentos can be displayed.

Facility Amenities
Southlake Village has been built to provide a warm, comfortable, state-of-the-art environment for 192 residents. Our amenities include:

  • Full air conditioning
  • No more than two residents per bedroom (many rooms overlook Sunnyhill Park)
  • No more than two residents sharing a washroom
  • A secure landscaped courtyard
  • Outdoor balconies on every level
  • Attractively designed and decorated dining rooms on every floor
  • Separate activity rooms, living rooms and lounges on every floor
  • Bathing facilities with a choice of therapeutic tub or shower
  • Main floor chapel, café, hair salon/barbershop and private family dining room (for special family events)
  • Auditorium for large parties and activities

Co-Payment Fees & Personal Tax Credit

Co-Payment and Funding Information
The Ministry of Health and Long-Term Care provides funding for long-term care facilities. The amount paid by residents for their accommodation is called a “co-payment”, which varies according to the type of accommodation. The Ministry sets maximum accommodation co-payment rates. While the co-payment rates change from time to time, the following table shows the co-payment rates effective July 1, 2003.

Government Subsidies
If your income is not sufficient to pay the accommodation co-payment rate for the long-stay program, a subsidy is available to reduce the basic monthly rate.

Residents with lower incomes or residents who have spouses or partners who are living in the community may be eligible for an additional subsidy called the “Exceptional Circumstances Reduction”. For more information, we recommend that you refer to the government handbook called “Accessing a Long-Term Care Facility”, or contact your local Community Care Access Centre, or speak directly to the Business Manager at Southlake Village.

Canada Customs and Revenue Agency
Being cared for in a nursing home is considered to be a medical expense for a person with a disability and as such may qualify you for a personal tax credit. Please seek professional guidance or contact your local Canada Customs and Revenue Agency office for details. You will need to familiarize yourself with the taxation form T2201 Disability Tax Credit Certificate, which can be pre-approved any time during the year by completing and sending it to your local taxation office.

Volunteering at Southlake Village

Southlake Residential Care Village has an active volunteer program providing support to our Life Enrichment team in many areas, such as:

  • Visits with Residents
    –  Play games, read, share experiences

  • Help with activities
    –  Games, crafts, parties, religious services, fitness, E-mailing

  • Assist with Wheelchairs
    –  To activities, courtyard, outings, to meals, etc.
