Patient Name Input Field
Please refer to “for the Account of xxxxx” on your Southlake Statement. This name may not always be the patient’s name who received care at Southlake. It may be the person responsible for the account payment.

Statements with Multiple Account Numbers
Patients who receive a statement with charges on more than one account can make a single payment through our online system and avoid the cost of paying a handling fee for each account transaction. To make a single payment for multiple accounts, please enter each account number separated by a comma. Example:  xxxxxx, xxxxxx, xxxxxx
