Palliative Care Program
At Southlake Regional Health Centre, we are committed to delivering quality, end-of-life care while providing our patients with the complete respect and dignity they deserve.
What is Palliative Care?
Palliative Care is the term used to describe the compassionate treatment of individuals and families who are facing a life-threatening illness. At Southlake, palliative care practices are based on providing each patient and their loved ones with respect, dignity, and an individualized care plan that meets their specific needs in symptom management, preparation for end of life, grief, and bereavement.
The focus of palliative care is to lessen the severity of the disease symptoms rather than seeking a cure. Palliative care strives to maintain or enhance a patient’s quality of life.
Who provides Palliative Care?
We have a compassionate team of healthcare professionals who have received specially training in palliative care practices. This team includes physicians, and advanced practice nurse, nurses, a social worker, a pharmacist, a physiotherapist, a chaplain, volunteers, dietitian and other professionals whose expertise may be required. However, the most important part of this team is the patient and his/her family.
The Palliative Care Unit
Our eight-bed unit includes four private rooms and two semi-private rooms. In order to provide patients and family members with as much comfort and convenience as possible, our facilities on the unit also include a kitchen, a lounge/dining area, and quiet room and an open terrace garden.
How to Access our Unit
After reaching an understanding and a discussion of their medical condition with their family and physician, patients may be referred to the Palliative Care Unit by a physician, nurse, social worker, Community Care Access Center Case Manager, or family. All referrals are to be made to our Advance Practice Nurse at 905 895-4521 ext 2788.
