This program provides 24-hour coverage to the Emergency
Department and provides assessment, short-term crisis counseling
and referrals to community agencies. In addition, this service includes
assessment and consultation to inpatient units and family physicians.

Our in patient unit currently consists of 24 beds and
provides short-term assessment, stabilization, and symptom reduction
to individuals with mental health issues. Individual, group and
family education is also provided.

Partial Hospitalization
This outpatient program is designed to provide clients
with assessment and stabilization of their psychiatric illness (e.g.,
depression, anxiety). The program offers flexible hours and allows
the patient to return to their home each night. Intensive individual
and group treatment, including family education, is provided. A
psychiatrist's referral is required.

Day Program
The Outpatient Day Program is offered to individuals 18 years of age and over. The program can last up to five weeks and is designed to provide group sessions to individuals with a psychiatric illness (e.g., depression, anxiety). Clients are encouraged to explore life skills, goal setting, problem solving, community resources, stress management, etc. A family support group is also offered to provide education and support. A physician's referral is required, or a referral may be facilitated by a Community Crisis Worker.

Brief Therapy Clinic
This outpatient program is designed for clients who are
18 years of age or older and have been diagnosed with a major psychiatric
illness. The program offers individual and group assessment and
treatment through short and medium term therapy sessions. A physician's
referral is required.

Schizophrenia Clinic
This outpatient program is designed to provide assessment
and treatment planning for individuals living with schizophrenia/psychotic
illness. The program includes individual, group and family education,
as well as medication administration and monitoring. A physician's
referral is required.

Psychogeriatric Assertive Community Treatment Team (PACTT)
The objective of PACTT is to provide intensive services for people over the age of 60 with severe mental illness who reside in York Region. The primary goals of PACTT are to:
- lessen or eliminate the debilitating symptoms of mental illness and to minimize or prevent recurrent acute episodes of the illness
- meet client needs and enhance their quality of life
- improve functioning in geriatric social activities
- lessen the family’s burden of providing care
- decrease the number of hospital admissions, re-admissions and bed days, emergency visits and crisis service contacts
The current findings of this type of service delivery is that clients have fewer admissions to hospital, and for those who are re-hospitalized, they experience significantly shorter hospital stays. Also, clients spend more time in independent living situations, experience more positive social relationships, express greater satisfaction with life and are less symptomatic.
Potential clients can be referred to PACTT through self referral, their community psychiatrist or family doctor, hospital staff, professionals from other agencies, a family member or a friend.
PACTT delivers treatment, rehabilitation and support services in the client’s choice of environment. This outreach program is offered 24 hours a day, seven days a week and is supported by an experienced team of health care professionals consisting of nursing, occupational therapy, social work, a peer support worker and a psychiatrist.

Geriatric Mental Health Outreach Program
Members of the Geriatric Mental Health Outreach team
visit the client's place of residence and provide assessment, consultation,
short term case management, education and follow-up to the elderly
who suffer from a serious mental illness. Emphasis is placed on
utilizing available community resources and services to ensure that
the client and caregivers receive adequate care and support. The
program also provides recommendations for treatment and care to
Long Term Care Facilities or family physicians. A physician's referral
is required.
To access the program, clients must:
- be at least 55 years of age
- suffer from a serious mental illness
- have a major behaviour problem and are at risk of harming themselves
- have difficulty with recent memory loss
- live in Southlake Regional Health Centre's service area

Assertive Community Treatment Team (ACTT)
This outreach program is centered in Georgina, Ontario. It is offered 24 hours a day, seven days
a week and supported by an experienced team of health care professionals
consisting of nursing, occupational therapy, social work, a peer
support worker and a psychiatrist.
The objective of the ACTT is to provide intensive services for
people with a severe mental illness who reside in Georgina. The
primary goals of ACTT are:
- lessen or eliminate the debilitating symptoms of mental illness
each client experiences and to minimize or prevent recurrent acute
episodes of the illness
- meet the client's basic needs and enhance their quality of life
- improve functioning in adult social activities and employment
roles
- lessen the family's burden of providing care
- decrease the number of hospital admissions, re-admissions and
bed days, emergency visits and crisis service contacts
The ACTT delivers treatment, rehabilitation and support services
in a client's choice of environment.

Community Treatment Order (CTO)
A community treatment order is a new, alternative form
of treatment for people with serious mental illness. It is funded
through the Ministry of Health and Long Term Care and is sponsored
by Southlake Regional Health Centre. This form of treatment allows
the client to continue living in the community while receiving support
and treatment specified in a plan that the client/family and clinicians
draw up together. The focus of the community treatment order is
to keep the individual well in the community and reduce hospitalization.
