Release of Information

In order to improve service and provide coordinated release of information throughout Southlake, all Release of Information (ROI) requests for personal health information, diagnostic imaging reports, and/or images (CDs &/or films) can be picked up in Health Information.

Requests for images and/or reports:

CDs &/or films requests must be placed with the ROI team Monday to Friday between the hours of 8:00 a.m. – 4:00 p.m.  These must be picked up by the requestor.  If a family member or friend is picking up on their behalf, they must have a signed letter from requestor.

Physician requesting faxed reports are asked to call extension 2315.

Exceptions:

After hours patient transfers to other health care facilities, clinical trials, images requested for education/teaching, and specialized imaging requests for use with medical devices will continue to be managed by DI staff. Please contact extension 2436.
