What is Telehomecare?
Telehomecare is an initiative designed to improve care for patients with heart failure and chronic obstructive pulmonary disorder (COPD).
The focus of Telehomecare is preventative care and self-management of chronic disease through education and health coaching. Telehomecare provides patients with the tools to be able to manage their own health and stay well at home.
Telehomecare relies on the use of supplied monitoring equipment in a patient’s home, including a blood pressure unit, weight scale and an oximeter to measure oxygen saturation. Through the use of the patient’s home telephone or an Internet connection, Telehomecare Registered Nurses are able to remotely monitor each patient’s health status on a daily basis, and work with the patient to set healthy living goals while learning how to stay healthy at home. The program involves one home visit by one of our Telehomecare Nurses followed by telephone coaching and remote monitoring. The length of the program is up to 6 months.
This program is open to any patient with a diagnosis of heart failure or chronic obstructive lung disease, who is willing and capable of partnering in their own healthcare to learn self-management tools for their disease.
Participants must also meet the following criteria:
- At least one emergency visit and/or hospital readmission related to chronic heart failure, or chronic obstructive lung disease, in the last 12 months, or have been newly diagnosed
- Patient or family caregiver capable of using simple in-home monitoring equipment on a consistent basis
- Patient or family able to provide informed consent to participate
- Patient lives in a residential setting with a phone line or an active internet connection provided through Bell, Rogers, Eastlink, Shaw, or Cogeco
This program is open to any patient with a diagnosis of heart failure or COPD and who lives in the catchment areas of Southlake Regional Health Centre (Newmarket, Aurora, Georgina) and Stevenson Memorial Hospital (Alliston and New Tecumseth areas).

Healthcare providers looking to refer patients to Telehomecare are asked to complete the Telehomecare Referral Form and fax the completed form to 905-830-5980.
