 Complex Medical Rehabilitation
The Complex Medical Rehabilitation program, incorporates
both complex continuing and neurological/general rehabilitation
on one integrated 35-bed patient care unit. The program utilizes
a multidisciplinary approach to providing care which involves the
patient and their family, the physician and primary nurse, along
with the expertise of the chaplain, dietitian, discharge planner,
pharmacist, social worker, speech language pathologist, physiotherapist,
occupational therapist and recreation therapist as required.
Patients and families are involved in establishing the care plan
through individual goal setting with team members. Patients are
encouraged to take an active part in their daily care and rehabilitation,
and to be up, dressed and participating in activities that promote
their recovery. The special needs of our patients are well supported
by an active volunteer program.
Neurological
and General Rehabilitation
Our 15-bed unit offers active rehabilitation to patients recovering
from a neurological condition (e.g., stroke) or a serious medical
or complicated surgical event. Our goal is to promote the best possible
functional independence of our patients. The average length of stay
is 27 days depending on established goals and the nature of the
underlying condition. On discharge 85% of our patients successfully
return home.
Complex
Continuing Care
Our 20-bed unit provides nursing care for patients who require
24-hour nursing care but no longer require constant monitoring and
treatment. The multidisciplinary team assists patients in achieving
their best possible function and supports patients and their families
in identifying the most appropriate discharge destination (e.g.,
nursing home, discharge home with assistance, etc.). The average
length of stay for patients whose needs can be supported in the
community is 40 days.
