The Cardiovascular Chronic Disease Prevention and Management Program is the essential next step in Southlake's Cardiac Care program and consists of several specialized services designed to improve the overall cardiovascular health of individuals who have coronary artery disease or other cardiovascular events or who have peripheral artery disease, are at high risk due to diabetes or have experienced a Tia or stroke with minimal deficits.  A physician referral is necessary to participate in this program.

Heart Function Clinic

The purpose of this Clinic is to allow close monitoring of patients with heart failure to optimize quality of life and prevent hospitalization. Most patients visit the clinic every three to four months and are assessed by a registered nurse, advance practice nurse, or a cardiologist.

The emphasis is on health teaching to maximize compliance to healthy lifestyle, early intervention of potential problems, and ongoing assessment for progression of heart disease. Referrals are through a Cardiologist’s office or while a hospital inpatient. The clinic operates Monday to Friday.

Cardiovascular Chronic Disease Prevention and Management

Participation in the Cardiovascular Chronic Disease Prevention and Management Program is essential to preventing recurrence of cardiovascular events.  Our team provides participants with  the necessary tools, skills, information and support to implement positive life long health behaviour change.  Our inter-professional team consists of cardiologists, registered nurses, exercise therapists, a social worker, a dietitian, and pharmacy support, who work with participants to personalize the program.

Each participant completes a treadmill stress test with our team prior to starting the program to determine the level of exercise that is safe for them.  During this visit the participant comes prepared to exercise and must bring along their medications and health card.  Cardiovascular history, clinical measurements and recent symptoms are reviewed at this time.  A follow up appointment is booked about one week later consisting of education about key safety information and supervised exercise called a track trial.  This group session is by appointment only and runs on Thursdays from 10:30- 12:30.   Following the track trial, participants attend 90 minute weekly sessions of combined education and exercise.  These sessions are supervised by exercise therapists who will monitor and support them through progression of individualized activity.

Extensive education regarding exercise, healthy eating, risk factor modification, stress management, cardiovascular medications, smoking cessation, weight management and more is provided in group sessions. Patients also have the option of meeting one-on-one with a dietitian or the social worker if required. The program is 6 months long.

Lipid Clinic

The Lipid Clinic provides medical consultation and treatment as well as comprehensive health behaviour change education and follow-up care for high risk patients with elevated and/or difficult to treat cholesterol or triglyceride levels.

Patients come in to meet with our endocrinologist for an initial assessment where medications may be initiated or changed as required. Group education sessions regarding health behaviour change is provided by our dietitian.  Individual dietary counseling may be arranged if required.  One to two follow up appointments may be required prior to returning to the care of the family practitioner.

Patients are instructed to have repeated cholesterol levels done at least one week prior to their next appointment and a lab requisition is provided. This clinic operates on Monday mornings.

24-Hour Ambulatory BP Monitoring

*This service operates from the Tannery 465 Davis Dr. Suite 404

A 24-hour ambulatory Blood Pressure monitor provides valuable information for patients with elevated blood pressure, fluctuating blood pressure readings, or to rule out white coat syndrome. The patient wears the monitor for 24 hours with values measured every half-hour during the day and every hour overnight. A report is generated and sent back to the referring physician. Appointments are booked Monday to Thursday.
