Cardiac Prevention and Rehabilitation is an essential part of Southlake's Cardiac Care program and consists of several specialized services designed to improve the overall heart health of individuals who have heart disease or are at high risk of developing heart disease. A physician referral is necessary to participate in these programs.
Heart Function Clinic
The purpose of this Clinic is to allow close monitoring of patients with heart failure to optimize quality of life and prevent hospitalization. Most patients visit the clinic every three to four months and are assessed by a registered nurse, advance practice nurse, or a cardiologist.
The emphasis is on health teaching to maximize compliance to healthy lifestyle, early intervention of potential problems, and ongoing assessment for progression of heart disease. Referrals are through a Cardiologist’s office or while a hospital inpatient. The clinic operates Monday to Friday.

Cardiac Rehabilitation

Cardiac Rehabilitation is provided to assist individuals with heart disease or those at high risk of developing heart disease with information and support to implement heart healthy lifestyles. Our multidisciplinary team consists of cardiologists, registered nurses, kinesiologists, a social worker, dietitians, and a pharmacist, who work with patienst to personalize the program to suit individual needs.
Each patient has an hour long, one on one intake/orientation session with a registered nurse; the patient history is obtained and patient questions can be answered. Prior to starting the program each patient is assessed by our cardiologist and kinesiologist during a treadmill stress test. This determines the patient’s current functional capacity and determines their exercise prescription. Participants attend twice weekly exercise sessions, where they will be supervised by a kinesiologist who will monitor and support them through progression of activity.
Extensive education regarding healthy eating, risk factors, stress management, and cardiac medications is provided in group sessions and held weekly for four weeks. Patients also have the option of meeting one-on-one with a dietitian or the social worker. The length of the program depends on the individuals needs but cannot exceed six months.

Lipid Clinic

The Lipid Clinic provides comprehensive education, medical treatment, and follow-up care for patients with elevated and/or difficult to treat cholesterol or triglyceride levels.
Patients have a risk stratification, base line assessment, and health teaching done by a registered nurse. A dietitian then provides a one-on-one session, which focuses on the patient’s dietary habits, as well as answering questions and providing supplementary handouts.
An endocrinologist provides medical support. Medications may be initiated and titrated as required. Patients are instructed to have repeated cholesterol levels done one week prior to their next appointment and a lab requisition is provided. This clinic operates on Monday mornings.

24-Hour Ambulatory BP Monitoring
*This service operates from the Tannery 465 Davis Dr. Suite 404
A 24-hour ambulatory BP monitor provides valuable information for patients with elevated BP, fluctuating BP readings, or to rule out white coat syndrome. The patient wears the BP cuff for 24 hours with values measured every half-hour during the day and every hour overnight. A report is generated and sent back to the referring physician. Appointments are booked Monday to Thursday.
