Cardiovascular Chronic Disease Prevention and Management Program
Participating in a cardiovascular rehabilitation program is integral to preventing disease progression and recurrence of vascular events. This 6 month program is designed for those who have experienced vascular diseases like cardiac events, TIA, stroke with minimal deficits, peripheral artery disease and high risk diabetics. We will help participants learn to enhance and maintain their health and improve their quality of life by assisting them to make and maintain health behavior changes. All participants complete an exercise stress test at the start and completion of service. A comprehensive individualized program of exercise and education is provided. Participation in once weekly 90 minute sessions as well as home exercise is expected. This program is fully supported by a highly skilled inter-professional team.

Cardiology Clinic
The Cardiology Clinic is designed for individuals referred by their family physician or through an emergency department who require urgent assessment of chest pain or other serious cardiac related symptoms not yet diagnosed. The visit includes an ECG and a consultation with a cardiologist. The visit may also include an Echocardiogram and / or Regular Exercise Treadmill Stress test. Further diagnostic tests may be ordered and arranged as required. Follow-up will also be arranged as necessary. The referring physician will receive a dictated note. Individuals should plan to be at the clinic for approximately 2 hours.

Heart
Function Clinic
The program is designed to meet the ongoing needs of patients with advanced congestive heart failure. A physician, nurse practitioner, and a registered nurse work together to provide detailed assessment, treatment and education. The goal is to improve the patient's quality of life by providing information and support that allows them to recognize problems before their condition worsens.

Clinic
This program is designed to counsel, educate and treat high risk patients with dyslipidemia. The program is provided by an inter-professional team and includes an assessment of risk factors and follow-up to optimize treatment, as well as individual counseling and group education sessions.

Pacemaker
Assessment/Follow-Up Clinic
The focus of the program is to provide ongoing assessment to individuals with permanent pacemakers as well as programming and monitoring pacemaker batteries to ensure safe functioning. Pacemaker follow-up visits also provide an opportunity for registered nurses trained in cardiac pacing to provide valuable education to patients and their family.
