The Cardiovascular Chronic Disease Prevention and Management Program is the essential next step in Southlake's Cardiac Care program and consists of several specialized services designed to improve the overall cardiovascular health of individuals who have coronary artery disease or other cardiovascular events or who have peripheral artery disease, are at high risk due to diabetes or have experienced a TIA or stroke with minimal deficits. A physician referral is necessary to participate in this program.
