Interprofessional Collaboration
Southlake has made a commitment to build a culture of interprofessional care by including this commitment in its 2013-2018 Strategic Plan. Southlake believes creating a culture of interprofessional care is imperative to help meet our organizational vision to create shockingly excellent experiences.
Interprofessional collaboration (IPC) is the developing and maintaining of effective working relationships among learners, practitioners, patients and families, and communities to enable optimal health outcomes. [Canadian Interprofessional Health Collaborative, 2010]

Elements of IPC include:

  • Shared leadership
A National Interprofessional Competency Framework helps health providers understand more about interprofessional care.
Southlake has established an Interprofessional Practice and Education Council (IPEC), whose mandate is to foster a culture where IPC becomes the standard in all aspects of Southlake operations, including clinical practice, teaching and learning, and research.

IPC example at Southlake: The Arthritis Program

The Arthritis Program (TAP) is an award-winning interprofessional model of care. TAP brings together members of its team to provide patients with comprehensive diagnosis, assessment, education, and treatment of their arthritis with the ultimate goal of self-management and empowerment.

Based on its success, in 2009, TAP won the University of Toronto Centre of Interprofessional Education’s inaugural Award of Merit for Role Modeling Interprofessional Collaboration for Patient-Centred Care for Learners.

In 2009/2010, the Canadian Initiative for Outcomes in Rheumatology Care, through the Canadian Rheumatology Association, awarded TAP a grant to create a training program to teach its model of care to others. The program has been successful in providing training to many teams from across Canada to further interprofessional knowledge, skills, and attitudes.

On behalf of the Alberta Health Services Arthritis Program  in Edmonton , we’d like to thank you, the Southlake Interprofessional Training Program, for taking the time to come to Edmonton.  It was amazing to have all of us in one room and realize how much we all share and that we aim to grow in a similar direction.  Thank you for facilitating this process.  Even over the past week our team has started to have more dialogue showing how we are becoming more comfortable in initiating communication with each other.  This interprofessional workshop was especially meaningful because of your understanding of the work we do.  I feel our group will benefit from your teaching and experience to enable us to create something of our own.
