Nurse-Led Outreach Team
The Nurse-Led long-term care home Outreach Team at Southlake consists of a Clinical Nurse Specialist and Nurse Practitioners who have partnered with the following Long-Term Care Homes to provide timely care to residents that otherwise might have been sent to hospital:

Their goal is to collaborate with residents, families and long term care teams to reduce avoidable transfers to the Emergency Department. The team can provide telephone consultation, scheduled visits, assessments and more. The team works to bridge the gap between hospital and community by improving communication and collaboration between hospital and long-term care home.
The long-term care home is encouraged to call for:
- Considering a transfer to Emergency Department
- Signs and symptoms of infection
- End stage dementia/Palliative
- Feeding tube problems
- Dehydration/Nutrition
- Behaviour changes
- Urinary issues
- Wound Care
- Constipation
- Pain
The team is available Monday to Friday from 8:30 a.m. to 4:30 p.m.
