Nurse-Led Outreach Team
The Nurse-Led Outreach Team at Southlake provides timely care to residents of long-term care homes that otherwise might have been sent to hospital.
The Team, consisting of a Clinical Nurse Specialist and Nurse Practitioners, collaborates with residents, families, and long-term care teams to reduce avoidable transfers to the Emergency Department. They can provide telephone consultation, scheduled visits, assessments and more.

The long-term care home is encouraged to for consultation on the following:
- Considering a transfer to Emergency Department
- Signs and symptoms of infection
- End stage dementia/Palliative
- Feeding tube problems
- Dehydration/Nutrition
- Behaviour changes
- Urinary issues
- Wound care
- Constipation
- Pain
The Nurse-Led Outreach Team is available Monday to Friday, 8:00 a.m. – 6:00 p.m.
