Job Summary:

This position encompasses working within community geographic Interprofessional Palliative Care teams to ensure the delivery of effective, high quality care to patients living with a life-threatening illness and their families.

The Clinical Nurse Consultant (CNC) will provide mentorship and support to primary clinicians and other members of the health care team to enhance palliative care knowledge and capacity within the community. As Lead Educators for Hospice Palliative Care in the community the CNC will provide education to health care professionals across the Central LHIN.

As a member of HPC Teams for Central LHIN, the CNC will participate in the provision of a 24/7  on call consultation service which includes an appropriate level of physician back up support.

Qualifications

  • Certificate of Competence from the Ontario College of Nurses
  • Baccalaureate in Nursing (Masters preferred)
  • Additional educational preparation as Clinical Nurse Specialist and/or Oncology/Palliative Care certification, example CNA Certification in Hospice Palliative Care Nursing
  • Proven experience with direct care in palliative care with patients
  • Member of RNAO with current malpractice and legal coverage
  • Valid Driver’s license, own vehicle
  • Minimum of 3 years’ experience in palliative care
  • Experience in delivering hospice palliative care education to interprofessional teams
  • Ability to work both independently and cooperatively with other members of the interdisciplinary and primary care teams
  • Able to work in a self- directed environment
  • Excellent clinical decision making skills with an expertise in palliative pain and symptom management
  • Demonstrated knowledge base of pharmacological and non- pharmacological interventions for pain and symptom management
  • Highly developed communication and interpersonal skills
  • Problem solving, critical thinking and analysis skills
  • Demonstrated proficiency in teaching, coaching and mentorship of individuals and groups
  • Excellent computer literacy – Microsoft Programs, e.g. Outlook, Word, PowerPoint
  • Comprehensive knowledge of community health services in the Central LHIN
  • Excellent time management skills with the ability to manage competing priorities
  • Well-developed oral and written communication skills     
  • Proven ability to attend work regularly as per the Attendance Support Policy #HA30 and Procedure #HA35.
