As a key member of Southlake’s Telehomecare Team, the RN provides remote patient monitoring, health coaching, and disease-specific education to patients. The Telehomecare RN will remotely monitor and coach patients with complex chronic diseases, including Chronic Obstructive Pulmonary Disease and Heart Failure. Telehomecare is based on a collaborative model of care that focuses on enabling the patient to better manage his/her chronic disease through the support of monitoring and coaching. The Telehomecare RN will work in collaboration with the patient’s circle of care.

Required Qualifications:

•       RN certificate of registration from the College of Nurses of Ontario, BScN preferred

•       Strong knowledge base and previous experience working with patients with complex

•       chronic conditions (i.e. CHF, COPD, Asthma, Diabetes, and Mental Health Disorders)

•       Certificate in Adult education, health coaching, patient self-management and case management skills preferred

•       Certified Respiratory Educator (CRE) designation preferred

•       Strong critical thinking and problem solving skills

•       Effective communication and listening skills

•       Basic computer skills

•       Ability to use Internet Explorer, Mozilla Firefox and Microsoft Office applications

•       Working knowledge of applicable occupational health and safety legislation

•       Working knowledge of privacy legislation

•       Knowledge of organizational policies and procedures

•       Use technology and best practices to remotely monitor patients

•       Multi-task and work in a fast paced environment

•       Proven ability to attend work regularly as per the Attendance Support Policy #HA30 and Procedure #HA35
