Job Summary:
Working collaboratively with the Emergency Department Team, Adult Crisis Workers, GEM , and Care Partners, the Social Worker will provide intensive support to older adults with multiple chronic co-morbidity and complex care needs and their caregivers to transition safely from the Emergency Department to recovery at home.

Duties and responsibilities:

  • Provide direct support, therapeutic interventions, education and counseling to patients, families and caregivers with complex medical needs.

  • Conduct a targeted Social Work assessment to guide interprofessional care that is reflective of the values, culture, and beliefs of the patient, family, and caregiver.

  • Build effective collaborative working relationships with the Interprofessional Team and stakeholders, including Adult Crisis Team, GEM , Spiritual Care, Emergency Department and Inpatient care partners, and community care partners.

  • Foster relationships and support communication across the circle of care during the ED admission.

  • Liaise and consult with care providers to leverage information and inform safe  transitions across programs, services, community, and primary care.

  • Maintain linkage with the patient, caregivers, community partners and Primary Care as required to support transition home.

  • Participate in practice evaluation and research activities to improve care for an aging population and caregivers and inform health quality improvement and efficiency across systems of care.

  • Work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of Southlake Regional Health Centre

  • Participate in knowledge transfer activities including: teaching and supervision of students, Social Work meetings, Social Work consultation groups and maintain all requirements for registration with OCSWSSW.

  • Maintain work load measurement and data collection as required

  • Knowledge of the range of service options available for the designated client population both within the hospital and community setting.

    Qualifications:

  • Masters of Social Work from an accredited university

  • Current membership with  OCCSWSSW (RSW)

  • Membership with OASW an asset

  • Previous experience working with target population: Frail older adults and caregivers.

  • Experience working with care partners across the hospital and community who are serving frail older adults and caregiver.

  • Demonstrated organizational skills to set priorities in a fast paced environment.

  • Computer skills required; ability to manage electronic recording, workload and meet submission timelines.

  • Demonstrated knowledge of the impact of care transitions across the acute, sub-acute and community sector

  • Knowledge of legislation and guidelines relevant to practice, including Healthcare Consent Act,  child welfare, elder welfare, social determinants of health, and social accountability in healthcare.

  • Knowledge of collaborative models of care, including Health Links.

  • Excellent written and verbal communications skills.

  • Effective counseling skills.

  • Proven ability to attend work regularly as per the Attendance Support Policy #HA30 and Procedure #HA35
