Heart Function Clinic

The Heart Function Program provides care to people who have cardiomyopathy and congestive heart failure. The program is designed with a focus on patient and family engagement, education, and strategies to improve quality of life. This Clinic is designed to provide rapid access to world-class heart failure care in an out-patient setting.

The Clinic's interprofessional team provides a detailed assessment, education, and a plan of care. The team includes:

  • Cardiologists (Doctors who specialize in treating the heart)
  • Nurse practitioners (nurses who have advanced training in the management of heart failure)
  • Nurses
  • Cleriical Support

Patients and their family are also important members of this team. Together we design a plan of care that will meet the patients needs.

A referral from a doctor or Nurse Practitioner is required to attend the Heart Function Clinic.

The clinic operates Monday to Friday, 8:00 a.m. – 4:00 p.m.

Patients should check in at the Welcome Centre, located on on Level 4 of the Medical Arts Building, before they go to their appointment.
