Meet Your Cardiovascular Prevention and Rehabilitation Team

You are in good hands. Our team of rehabilitation specialists areleaders in delivering cardiovascular rehabilitation and secondary prevention programs.

Over the next several months, we will work with you to develop your own personal program of exercise and education. This will help improve your cardiovascular fitness and muscular strength, In order toreduce your chance of having anothercardiovascular event.

See below for skills brought to the program in support of the well being of the Program participant.

Registered Nurse- Our Registered Nurses are part of the Exercise Therapy team and have a background in specialties such as Diabetes Education and Cardiac Nursing. They provide exercise prescriptions including exercise progression and home programs. They also design and supervise sessions, motivational manner and monitor participants to ensure safety and comfort. 

Registered Kinesiologists - Our Registered Kinesiologists are part of the Exercise Therapy team. They provide provide exercise stress testing and prescriptions including exercise progression and home programs. They also design and supervise exercise sessions, provide education sessions, assist participants in their wellness journey by addressing key risk factors in a supportive, motivational manner and monitor participants to ensure safety and comfort.

Social worker - The social worker provides short-term individual and group counselling support to participants. Counselling addresses common emotional and life challenges that people with a chronic illness face (including stress management), and helps to identify individual strengths and develop healthy coping strategies to improve physical well-being.

Dietitian- The dietitian provides group education and individual nutrition assessments, counselling and follow-up with the goal of optimizing the participant’s cardiovascular health. Group sessions may be classroom based or a hands-on experience within a grocery store or community kitchen.

PharmacistA community-based pharmacist conducts education classes regarding prescription and non-prescription medications.

Administrative Staff - Our friendly and knowledgeable administrative staff will be your first contacts with our program. They will help to set up your initial appointment and answer questions you have prior to starting the program.
