Patients scheduled for an out-patient test, clinic visit, or
surgical procedure are encouraged to pre-register for their appointment
at least 48 hours in advance of their appointment, prior to coming to the Hospital.
Step 1: Pre-Register
- Complete the Pre-Registration Form and click “Register”
Please note:
You will be directed to separate, secure website called
“hospitalportal.ca”, operated by Infonium Inc. Infonium Inc. is
contractually bound to maintain Southlake patient privacy &
confidentiality as is required by the Personal Health Information
Protection Act, 2004. Neither Infonium Inc,. nor Southlake will use or
disclose personal details entered into the Pre-Registration Web Form
for any purposes that do not relate to operating the online
Pre-Registration process.
- Alternatively, you may print the completed form and fax it to 905-830-5809
In order to complete the pre-registration process, you will need to know:
- your basic personal information
- your doctor’s name
- appointment date and time
- emergency contact information
- your Health Card number
Step 2: Check-In
Upon arrival at the Hospital,
“Check-In” at any one of our Self-Registration kiosks, or visit one of
our Welcome Centres, where a Patient Access Associate will be happy to
complete your registration.
If you choose to use one of our Kiosks, you will be
asked to swipe your Health Card and verify the information that
appears on the screen. You will then be asked to proceed directly to
the area of your appointment.
Note:
In some cases, your information may require updating. Our
Kiosks will indicate if this is the case, and direct you to the closest
Welcome Centre or Courtesy Phone where someone will be available to
update your information.
If you are coming to the Hospital for a series of clinic visits, you do not need to pre-register before each appointment, provided you meet the following criteria:
- You complete the pre-registration process prior to your first appointment.
- Your registration information remains the same for the duration of your visits.

- East Entrance Main Lobby
- West Entrance
- Level 4, Medical Arts Building
- Emergency Room - Limited to Emergency patients only
Express Check-In Kiosk Locations:
- East Entrance Main Lobby
- West Parking Entrance
- Level 3, Medical Arts Building
Can’t Pre-Register?
Although pre-registration is quick and easy for all
patients, we understand that from time to time there may be exceptional
circumstances that do not allow for it. For patients who are unable to
pre-register for their appointment prior to coming to the Hospital,
please visit one of our Welcome Centres where one of our friendly
Patient Access Associates will be happy to assist you.
