"One-on-One" NUTRITION COUNSELLING: A PERSONALIZED PLAN

Program details:

         

  • learn how to eat properly, for long term: at home, at work, and on-the-go (away from home)
  • Achieve and maintain your ideal body weight & how to finally put an end to yo-yo dieting
  • Incorporate science-based dietary strategies to help you manage your cholesterol, triglycerides and blood pressure
  • Prevent and reverse heart disease
  • Optimize your intake if you have special dietary needs: vegetarian, vegan, etc.
  • Eliminate fat, sugar, carbohydrate cravings and emotional eating obstacles
  • Understand the role of calories, nutrients & minerals in diet & how to optimize intake for heart health
  • learn how to read food labels & become a smart "heart & waist-friendly" shopper at the supermarket
  • Discover strategies for optimum meal planning and how to revise your favourite recipes
  • 1, 2, 3 or 4 week sample meal and snack ideas containing "what & how much" to eat at all meals and snacks
  • learn strategies for heart-healthy dining away from home
  • Learn the "do's" and "don't" of nutrition supplementing
  • Nutrition educational materials at every visit on various topics related to food, nutrition and heart health
  • Support & guidance during all your visits to help you stay motivated and on track towards reaching your goals
  • Your food record, successes & challenges reviewed at every visit: new goals and realistic strategies will be established

Length of time:

  • One comprehensive nutrition and health assessment (60 minutes)

  • Several follow up sessions (30 – 45 minutes each) based on your need

  • Length of time between follow up appointments will depend on your goals and your schedule; this will be determined between you and the dietitian.

  • Unlimited telephone or email support for the duration of your 6 months at Cardiovascular Prevention & Rehab
