Heart Function Program
The Heart Function Program provides care to people who have cardiomyopathy and congestive heart failure. The program is designed with a focus on patient and family engagement, education, and strategies to improve quality of life. This Clinic is designed to provide rapid access to world-class heart failure care in an out-patient setting.
The Clinic's interprofessional team provides a detailed assessment, education, and a plan of care. The team includes:
- Cardiologists (Doctors who specialize in treating the heart)
- Nurse practitioners (nurses who have advanced training in the management of heart failure)
- Clerical Support
Patients and their family are also important members of this team. Together we design a plan of care that will meet the patients needs.
A referral from a doctor or Nurse Practitioner is required to attend the Heart Function Clinic.
