Post Discharge Clinic

The Discharge Clinic is one of the Adult Mental Health Out-patient programs. The team consists of Psychiatrists, a Registered Nurse and a Social Worker, who will provide a maximum of four follow-up appointments for patients recently discharged from the Mental Health In-Patient Unit.

Referral Process

  • Psychiatrist referral from the adult in-patient unit
  • Repatriated patients from our emergency department

This Program is Not Suitable For:

  • Patients with cognitive impairment as a sole diagnosis
  • Patients with substance abuse as a sole diagnosis
  • Patients that have a Psychiatrist with a follow up appointment within 4 weeks
  • Patients that are actively enrolled in an outpatient program


Services Available

  • Intake, assessment and treatment care plan completed with the patient by RN/SW in collaboration with the Psychiatrist
  • Medication reconciliation, assessment, medication teaching, administration as needed.
  • Review and facilitation of patient’s discharge plan, crisis plan, and health teaching and support for patient in their recovery
  • Facilitate referrals and integration to outpatient community programs for continuity of care
  • Assistance as needed in completion of forms such as ODSP, Drug cards
  • Potential participation with the Emotional Regulation Training Group
