Patient and Family Advisory Program

At Southlake we believe that the patient’s perspective is key in our ability to deliver exceptional healthcare services. The goal of the Patient and Family Advisory Program (PFAP) is for patients and their family members, care givers or chosen support persons to work in partnership with staff and physicians when discussions, plans, and decisions are being made about the delivery of healthcare services at Southlake.

What is a Patient and Family Advisor?

A person who, within the past five years, has been a patient or family member/caregiver or chosen support person to a patient at Southlake. Patient family advisors share their knowledge and abilities related to healthcare experiences, positive and negative, as a recent patient, family member, caregiver or chosen support person of a patient.  

The role of a PFA includes, but is not limited to, participating on a Council, Committee, or Working Group with Southlake staff and/or physicians to provide input and feedback on:

  • Programs, services, and facilities
  • Various quality improvement initiatives and projects
  • Policies, processes, and educational materials

 As a Patient and Family Advisor, you will contribute to creating the best experiences for patients.
