To enroll into optional benefits please download and complete the Benefit Enrolment Form
and submit to Human Resources. Full time staff are eligible to enroll within 30 days
of their full time hire date or the date of transfer to full time status.
Changes to your benefit coverage can be made within 60 days of a qualifying life event (marriage, birth of child, loss of spouse's coverage, etc.) by submitting the enrolment form along with supporting documentation of the life event to Human Resources. An open enrolment period is also held each November, allowing staff to enroll or make changes to their benefit coverage without a qualifying life event for an effective date of the following January 1st. Details regarding the open enrolment period will be communicated to staff via Southlake email address each fall.
