Partners in Care

At Southlake we work with a number of community-based healthcare partners to provide care and ease transitions from the Hospital to home and/or to community providers. From time to time the Hospital also hosts a number of initiatives of the Ministry of Health and Long-term Care and the Central Local Health Integration Network, providing these initiatives with space and some administrative support.

Below are links to some of our partners in care.

Community Care Access Centre (CCAC)
The Central CCAC provides a single point of access for:

  • one-stop information and referral for healthcare and community services
  • in-home healthcare & support services to eligible clients
  • placement of eligible clients in nursing homes and homes for the aged

Community Home Assistance to Seniors (CHATS)
Community Home Assistance to Seniors offers a variety of services including two that help smooth the transition from Hospital to home; Home at Last and Doorways to Care.

Home at Last provides reliable transportation home from the hospital and settling-in assistance by a certified Personal Support Worker.

Health Links
Ontario is committed to improving healthcare for seniors and people with complex conditions through the creation of Health Links. The Province of Ontario has identified 37 early adopter Health Links. Southlake Regional Health Centre and North York General Hospital, working closely with the Central Community Care Access Centre, are the coordinating sites for two Health Links in the Central Local Health Integration Network.

South Simcoe Northern York Region Heath Link

The South Simcoe and Northern York Region Health Link is focused on patients with complex medical conditions. These included frail, older adults, those with multiple chronic conditions, and people with mental health and addiction challenges.

Healthy Living Now
Healthy Living Now offers a 6-week self-management workshop called “Living a Healthy Life with Chronic Conditions.” This workshop empowers people to live well while dealing with conditions like diabetes, heart disease, arthritis, lung disease, and other chronic health issues. Southlake Academic Family Health Team
The Family Health Team can provide ongoing healthcare through a team of family doctors, registered nurses, and other healthcare providers, such as dietitians and social workers.
