Getting Started
Completion of the required documentation and payment of the the necessary deposits is required in order to ensure your placement isn't delayed.

Allied Health and Non-Clinical students must have completed an on-line application as part of their placement process. If you have not submitted an application, please visit Post Secondary Student Placements and submit the appropriate application, even if your placement has already been confirmed.

Please identify which section applies to you and follow these instructions:

Nursing/Paramedic GROUPS
Individual Students
Group/Faculty Leaders are required to bring these required documents to the Professional Practice department on the first day.
Students are required to bring these required documents to Professional Practice on their first day of placement.

 
icon Cash-128.png Paying the Deposit for ID Badge/Safety Pendant and Sterile Surgical Scrubs
Deposits are required in order to obtain your ID Badge, safety pendant and surgical scrubs*. The deposits will be refuned at the Completion of Placement upon return of these items. In addition to the amounts noted below, a non-refundable $2.00 processing fee will be applied.

Note: you must return BOTH the ID badge and the Versus safety pendant, and scrubs if used, within one month of your placement end date to receive a full refund! If your placement completion date is changed, please notify Professional Practice.

*Refer to the Scrubs List to see if surgical scrubs are required for your placement. If you are unsure about whether or not you require surgical scrubs for your placement, please select “Scrubs & Badge” for your deposit. If surgical scrubs are not required for your placement, the scrub portion of the deposit can be refunded.

Pay your Bill on-line as you will need it to successfully complete the transaction:


You will have the option to pay by Visa, Mastercard, or American Express.
  • Type in Badge or Scrubs and Badge in the "Account" field
  • Under "Invoice Date," enter today's date
  • Under "Patient Name," type in your name
  • Under "Contact Name" enter your school name
  • Under "Contact Phone Number," enter your phone number
  • Under "Amount," type in $40 for an ID Badge or $100 if you also require scrubs.
Ensure you PRINT THE RECEIPT and bring it with you on registration day in order to receive the items. If you are unable to pay in advance, please go to Facility Operations on your first day to pay by debit prior to going to Professional Practice to register. Facility Operations is located in office #1/823 “EOC” Level 1, West Building.

icon id badge Go to the Professional Practice Department on your First Placement Day to Obtain ID Badge/Safety Pendant 
  1. On the first day of your placement, bring the required documentation to the Professional Practice department:
Professional Practice is located on the 5th floor (south corridor) of the Southlake Village. 
Hours: Monday - Friday, 0800-1600 hours.The Southlake Village is the building located on the south end outside of the main hospital - the entranceway is directly across the road from the outside MRI trailer.
  1. Once all paperwork is received, Professional Practice will issue you an ID badge/safety pendant (mandatory) and access to scrubs (if required).

Orientation and Core Curriculum
You may be required to attend Orientation. Please refer to your student placement letter or introductory email to check if orientation dates were provided.

If orientation dates were provided or you have been instructed to attend orientation, please register for orientation.

Important Instructions:
 
Signing of the Student Agreement and Staff Assist Pendant Acknowledgement form is verification that faculty and students have reviewed the student orientation manual and core curriculum (completion of review is mandatory prior to commencing placement at Southlake):

Computer Access and Training
  • Nursing students - are required to attend computer training provided by Clinical Informatics (please e-mail computertraining@southlakeregional.org as per instructions in your student letter).
  • Allied health and non-clinical students: if you do not have your computer login instructions and passwords by the time you arrive at Southlake, you must contact the ICT Service Desk at (905) 895-4521 x 2246, ext. 1 to obtain your computer log-in instructions and passwords. You will receive individualized computer training from your clinical instructor/preceptor as required.

Illness/Injury

If, at any time during your placement, you are injured or contract a disease, please follow the process for Reporting an Injury or Illness.
