HIROC Checklist
A required checklist compiled to ensure you receive the proper orientation/education throughout your starting period with us. Please initial beside each topic once you have been introduced to it. Once the checklist is completed, sign and date the bottom of the form and return the completed check list to Human Resources to be kept in your employee file.

Many areas of the Hospital have department specific lockers. The Hospital also has a general locker room available to all staff. Please review our locker policy to see if you will require one and how to go about accessing the lockers.

In order to achieve a scent-free environment at Southlake we ask that anyone within the Hospital limit the amount of scented products used to support those who may have sensitivities towards perfumed products.
